Is your church interested in bringing a ministry team to work with Touched Romania outreaches?

1. Have your team leader contact us at raegan.glugosh@agmd.org

2. Fill out the Team Leader Application and email it to raegan.glugosh@agmd.org and you will be contacted shortly thereafter.

3. Once your team is approved and a date is established, your team leader will need to register with the Assemblies of God World Missions, MAPS Teams/CP&D office. The person to contact there is: Andrea Thee, Tel: 417-862-2781 ext. 2067, E-mail: mapsteams@ag.org

4. Each team member is required to fill out an Assumption of Risk and Insurance Election, Beneficiary Designation and Code of Conduct form. The Insurance List Report   needs to be filled out only once, by the team leader.  The Insurance Letter is to help explain what needs to be done with the forms. The forms, with original signatures, need to be sent back to the before the team leaves. The address that the forms need to be sent to is as follows:  AGWM Insurance, Attn:  Lori Bailey, 1445 N. 5. Questions regarding points #4 and #5 can be directed by phone or e-mail to Deanna Boneck (see contact information above).

6. As the dates of your team get closer we will be in e-mail contact with you regarding your missions trip ministry planning, budget, travel arrangements and other details.
